Dr. Jacobs believes financial clarity is part of good care. Here's everything you need to know about fees, insurance, and payment options before your first session.
Session Fees
Fees are discussed during the free consultation. A limited number of sliding scale spots are available for clients with demonstrated financial need — please ask.
Insurance & Superbills
Out-of-Network Provider
Dr. Jacobs is an out-of-network (OON) provider and does not bill insurance directly. Payment is due at the time of each session.
Superbills for Reimbursement
A detailed superbill (itemized receipt with diagnosis and procedure codes) is provided upon request. Many PPO plans reimburse 40–80% of out-of-network mental health fees. Submit your superbill directly to your insurance company for reimbursement.
How to Check Your Benefits
- 1Call the member services number on the back of your insurance card.
- 2Ask: "What are my out-of-network mental health benefits?"
- 3Ask about your deductible, co-insurance percentage, and any session limits.
- 4Confirm that outpatient psychotherapy (CPT code 90837) is covered.

- Zelle, Venmo, and bank transfer
- Check (in-person sessions)
Payment is collected at the start of each session. A receipt is provided for every payment.
Cancellation Policy
Dr. Jacobs asks for at least 24 hours' notice to cancel or reschedule a session. Late cancellations and no-shows are charged the full session fee. Exceptions are made for genuine emergencies — please reach out as soon as possible if something comes up.
Good Faith Estimate
Under the No Surprises Act, you have the right to receive a Good Faith Estimate of expected costs before beginning services. Dr. Jacobs provides this estimate to all new clients. If you have questions about your estimate, please ask before your first session.
